- We are located at 555 Black Oak Drive, Suite 400. This is in the Black Oak Medical Building. Once inside the building, you must take Elevator "C" in order to reach our suite. The other elevators in the building do not go up to the Fourth Floor. Elevator "C" is the furthest one from the front door in the downstairs lobby.
- Please complete your packet of patient forms and mail it to our office. If your appointment is within the next 5 days, you may complete the paperwork and bring it to your appointment.
- Be sure to come to the Renal Care office at least 15 minutes early.
- The day of your appointment, remember to bring your insurance cards with you.
- Our physicians will also need to know all of the medications you are taking. Please bring a list of these medications to your first appointment, or you can bring your bottles of medications to our office.
- When calling our office, a receptionist will ask you the nature of your call. This will ensure your questions/requests are sent to the right person. We have specialists in referrals, Medical Records, and Billing, as well as our Clinical staff.
- Calling ahead to make an appointment for a blood pressure check or a nurse visit will ensure we can set aside time to take care of your needs.
- If you have forms for your doctor to fill out, please let the receptionist know to ensure proper handling.
- Call your pharmacy and they will fax a request to us.
- Mail orders: please leave a message with us. We need to know if the order is to be faxed to the pharmacy or mailed directly to you. Leave any pertinent information such as ID#. Please allow 2 business days to process your request.
- You may mail requests for mail order prescriptions to our office.
- If you bring a refill request to the office, please leave it with our receptionist.
- If your request is urgent, please let us know.
