To request a copy of your medical records for either your own personal record keeping or to send to another facility of your choosing, please complete and sign the Authorization to Release Medical Information form with your signature. Send your request to the following address:

• This authorization is not needed if a Renal Care Consultants physician has referred you for laboratory or radiology testing, or to another physician for further treatment.
  
  
• Please allow the Oregon minimum standard of 30 days for request of records before inquiring about your request.
  
  
• It is not the policy of Renal Care Consultants to release information that did not originate from our physicians or staff. This is called third party releasing. Therefore, the records sent to us from outside sources (from consulting/referring physicians or transfer records) will need to be released from the originating office or hospital.
  

If you wish to review the health information in the Medical Records office, please make an appointment by calling (541) 734-3480. The office is open from 8 a.m. to 12:00 p.m. and 1:00 p.m. to 5:00 p.m., Monday to Friday.
