  • An updated medication list including all prescription and over-the-counter medications/vitamin supplements

  • Doses and how often you take the medication included on the medication list.

OR

  • All of your medication bottles

Without this information your provider cannot appropriately and safely alter  your treatment plan.

To request a copy of your medical records for either your own personal record keeping or to send to another facility of your choosing, please complete and sign the Authorization to Release Medical Information form with your signature. Send your request to the following address:

If you wish to review the health information in the Medical Records office, please make an appointment by calling (541) 618-4400. The office is open from 8 a.m. to 12:00 p.m. and 1:00 p.m. to 5:00 p.m., Monday thru Friday.

Billing

Please remember to bring your insurance identification card to your appointment. Renal Care will bill primary and supplemental insurance companies. Co-payment is requested at the time of your appointment.

If you do not have insurance, payment in full is requested on the day of your appointment. We accept Visa, Mastercard, and Discover. A minimum of $100 deposit is required with a prior arrangement for billing.

Finance charges accrue at 9% annually on all patient balances unpaid after 60 days of the first billing.
